Incomplete fuel combustion.
This is due to a air to fuel ratio imbalance, either the fuel system is delivering too much fuel into the engine or there is not enough clean air (oxygen ) a few things to look for:
Faulty injectors (injectors need attention at about 100.000 to 120 000 miles)
Faulty injector pump
Dirty air cleaner
Turbocharger or intercooler faulty
Problems within cylinder head, valves clogged up due to faulty EGR (exhaust gas recycling unit)

Is the car way down on power, and only pulls when your foot is flat down?
It will tick over but pulling away it is dead sluggish until your foot is buried?

If it's to do with the EGR, I'd get rid of it. It's an emissions bullshit idea which clogs up the inlet manifold/tract. Total waste of time. It can be disabled (called EGR delete in the car world), but the inlet side will need a good clean out. You'll be surprised at the extra performance and fuel economy you'll get by doing it. I've done it to my 1.9tdi PD engine and it's much happier. You may get issues with an emissions test, but if that's the case, just connect it back up.

Black smoke = fuel
Blue smoke = Oil
White smoke = Water/Coolant
Green smoke from dashboard = Electrics on fire. (had that on a Marina. Was everso pretty til i realised wtf was going on)
I'd chuck a 1 shot Redex thru it for starters. £1.99p will cure shitty injectors.
EGR valve is another common option to look at but secondary to the injectors imho

As Sam said,incomplete combustion
As kwack said different colour smoke, different issues
Unless your motor is brand new you will always get a degree of unburnt fuel on acceleration, quite normal
If you are down on power, then a new fuel filter is a good move,also check air filter as diesels need loadsa clean air (more than a petrol)
Also see if you can see if there is a filter on the engine breather, if so replace or dissconnect
Injector cleaner good idea but not sure Redex is man enough to be curative.its ok for maintenance but if you got shitted up injectors give an injection place a ring, Forte annd Tech 2000 do the do (cant buy em from Halfrauds)
also have a look at EGR sensor, usually on header pipefor spits below manifold, make sure wiring ok. Replacement only sure test for this one
finally check for splits in any of those wierd rubber link pipes going from air filter to other places (fuel pump,other pointless sensors ect )

I did Mart and it did a good job - however the benefits haven't lasted as long as the blurb suggested. I'm planning to put another can through before the MOT is due.

thanks all especially Kwackerz that gavew me a proper chuckle, I have done the air filter and the cabin filter (wow!) and I am on the second tank with redex in. It was a bit limp until I gave it the Italian job which definitely removed some warning lights

and freed up acceleration.
Thats about when the black smoke styarted too and the fuel filler cap open message and the engine management light LOL
I have the benefit of no one wanting to tailgate me even if I cut them up so thats a plus ;)
I like the car am not too stressed about it and plan on doing things bt by bit but would like to get this soirted before doing the suspensiuon bushes etc So I will try to figure out the fuel filter next and ask about a defap and remap which I believe bypasses the EGR rubbish
As for the MOT not sure the emissions would count as the FAP / EGR rubbish is an extra to the cat isn't it?

Try running it on white instead of red diesel geoff

thats about right and loads of smoke at the point of acceleration and that gets less if you hold a steady speed seems worse in the bigger gears 2/3/4
