Most* bike shops down here have a standard form like that, to the tune of about AU$1000. You drop / prang / bend the bike, you pay for it. The salesperson should point out the excess / conditions of loan when they give you the bike. If they don't, then they deserve a good whacking with a piece of two-by-four.
